ACCESS TO INFORMATION

In order to properly assess your needs and services required, Choice will require full access to your medical history. Please refer to the Medical History Release Form for further information. Choice requires that this form be fully completed and signed prior to any service arrangements being made or options discussed.

PROCESSING FEE FOR INVESTIGATION

Choice will require that the client pay a non-refundable processing fee in advance of commencing the process necessary to determine if, or what services may be provided to you.

Choice disclaims all warranties, express or implied, including but not limited to the operation, safety, condition, opinions, or services of any physician, health care provider, hospital, aircraft, motor vehicle, boat, other conveyance, lodge, inn, motel, hotel, resort, and/or products, services or opinions associated with such entities.

Choice does not warrant that the functions contained in the materials will be uninterrupted or error-free, that defects will be corrected, or that the Choice website or server that makes them available for use are free of viruses or other harmful components.

GOVERNING LAW:

This Agreement shall be governed by and construed in accordance with the laws of the Province of Manitoba, without regard for its conflict of laws provision and the federal laws of Canada applicable herein. The Parties irrevocably submit to the jurisdiction of the courts of Manitoba.

INVALIDITY

If any of the provisions contained in this Agreement are found by a court of competent jurisdiction to be invalid, illegal or unenforceable in any respect, the validity, legality or enforceability of the remaining provisions contained herein shall not be in any way affected or impaired thereby.

EXTENDED MEANINGS

Words expressed in the singular include the plural and vice-versa and words in one gender include all genders.

FURTHER AGREEMENTS

The parties understand that, as a precondition to Choice providing services to you of any kind, and from time to time as any services may be provided, additional specific and comprehensive agreements will be required including, but not limited to, release(s) of medical information for investigation and analysis, general liability release(s), and detailed travel, medical treatment and surgical plans. Such further agreements will be provided to you for consideration and independent review will be made available to you upon request and in any event in advance of any requirement for you to sign such agreements. All further agreements shall be construed as being an addition to earlier agreements, and do not negate or replace any of the earlier agreements.

PRICES, FARES AND RATES

All prices, rates or fares provided on the site or in brochures and associated materials are subject to change without notice. Special conditions may apply to the prices setout in the materials, advertisements and the website.

CURRENCY

Unless otherwise indicated, all dollar amounts referred to in this Agreement are in Canadian funds.

TRAVEL DOCUMENTS

All clients are responsible for the acquisition of their necessary travel documents which may include but is not limited to, passports, visas and the like. We will attempt to provide assistance where possible, however we make no implicit or explicit assurances or guarantees regarding a clients ability to receive their necessary documentation.
